 Health and Social Care Administrative Support Worker
 
An exciting opportunity has arisen to join a new team supporting both Health & Adult Social Care. The post holder will be part of a virtual team of Adult Social Care and Health staff supporting the early intervention of support for patients identified as being at risk of needing additional health and social care intervention.
 
You will provide administrative support to multi-disciplinary teams comprising of social care teams and health staff within a defined locality.
 
As part of this role, you will take on the administrative management of the regular multi-disciplinary team (MDT) meetings, and support in the completion of tasks arising from these meetings.
 
The post holder will help to facilitate the integration of health and social care by maintaining up to date recording systems for organisations and by providing information to members of the multi-disciplinary team.
 
You will be working across different computer systems in gathering relevant information to support staff in making decisions regarding early intervention with patients.
 
KEY TASKS:
 
You will be required to contribute to the coordination of the MDT meetings, this will include setting up the meeting, ensuring agendas are issued and capturing the notes of the meeting.
You will also support the delivery of health and social care plans for people who are newly discharged from hospital, discharged from a community assessment bed or who need support within their current home to prevent an unnecessary admission.
Tasks will, under delegation, include information and data coordination and effective communication.
