The Homecare Assessment and Reablement Service provides short term support that helps people return home from hospital, or prevent them from being admitted to hospital, and regain their independence. Reablement is a type of support that is short term which helps someone to remain independent, by giving them the opportunity, motivation and confidence to relearn or regain some of the skills for daily living that may have been lost as a result of illness, accident or disability.

Due to an expansion of the Home Care Assessment and Reablement Service, we are looking for someone who is able to effectively plan, manage and prioritise their work load and manage deadlines while completing Holistic assessments and  person centred reablement/ support plans and risk assessments and to have the ability to coach, mentor and support the reablement support workers in their performance and development and to effectively manage and motivate them


This is challenging and rewarding work providing intensive support for people in their own homes throughout Leicestershire for a short- term intervention.

You will work with Occupational Therapists and Physiotherapists, to help deliver an agreed Reablement plan, to help Service Users identify and achieve their goals relating to a range of social care tasks such as washing and dressing, showering, meal preparation and helping people engage in community activities. You will then work with Service Users to access long term support completing a care and support assessment and identifying their longer-term goals and needs or sign posting them to various voluntary or external organisations


You must have the ability to communicate effectively, both verbally and in writing. An NVQ 3 or equivalent and relevant experience working with adults in a social care setting is Essential. A good understanding of the relevant regulations and legislation and service Frameworks. The ability to work on a rota to meet the needs of the service and to always have access to a vehicle for work.
