Salary Range: £27,396 – 40,701 (plus market premium for level 3 Social Worker or AMHP)
    
Location: County Hall – Anstey Frith Building. Currently working from home but will be expected to attend the Bradgate Mental Health Unit, Glenfield Hospital.
    
An exciting opportunity has arisen for the appointment of an experienced Social Worker or Social Worker/AMHP within the Mental Health Hospital Discharge Team, to work with people who are predominantly aged 18-65 and have inpatient status at the Bradgate Mental Health Unit, Mental Health Rehabilitation Units, and Out of Area hospitals. We have associated link workers from the Mental Health Reablement and Assertive Inreach services, who support the work of the team.
   
We are an established, supportive team who co-produce safe, supportive discharge plans with people using the service and their informal support networks, incorporating neighbourhood, Reablement and Recovery models.
    
Applicants must be able to work using their own initiative when faced with high risk/challenging situations and be a supportive team player. You will receive regular 1:1 supervision and weekly group supervision with all team members.
   
Main Duties involve:
  • Care and Support Assessments (Care Act, 2014)
  • Mental Capacity Act (2005) assessments
  • Attendance at CPA meetings
  • Completing Social Circumstances reports
  • Safeguarding Adults and Vulnerable Adult Risk Management investigations
  • Multi-Disciplinary Team working
     
Essential Requirements:
  • Qualified Social Worker and Approved Mental Health Professional (or willing to train)
  • Registration with Social Work England
  • Excellent risk assessment and management skills
  • Prioritising work and meeting deadlines
  • Experience of working with people with a broad range of mental heath conditions
  • Working knowledge and experience of utilising relevant legislation including The Care Act (2014), Mental Health Act (1983) , Mental Capacity Act (2005), Human Rights Act (1998) and Equality Act (2010)
  • Working knowledge and experience of Recovery models, Personalisation, Asset and Strength Based Social Work, Neighbourhood and preventative strategies and solutions

The successful candidate will work Monday to Thursday 8.30 – 5pm and Friday 8.30 – 4.30pm but may occasionally be required to work out of hours to fulfil AMHP duties and responsibilities. Flexible working may also be considered.
