The Community Response Service helps people return home from hospital, or prevents them from being admitted to hospital, and supports people to regain their independence by providing a short term service which helps them to become as independent as possible by giving them an opportunity, motivation and confidence to relearn or regain skills for daily living that have been lost as a result of illness, accident or disability. As we are an expanding service, you will be part of something new and can help to steer the team towards transformation, while maintaining our high standard of care and making a real difference to the people and communities of Leicestershire.
As a coordinator you will be responsible and accountable for:
• Planning and programming care calls by using the Electronic Rostering and Monitoring system.
• Responding to all incoming referrals in a timely manner whilst ensuring individual needs are met through a personalised approach.
• Undertaking assessments of the need of the service users and their carers to provide a short-term care management service including the preparation, implementation and monitoring of individual care plans or appropriate sign postings to other early intervention and prevention services.
• Providing day-to-day supervision of CRS/HF Senior HCA's to ensure that all staff provide appropriate responses to referrals to the CRS/Home First.
• Ensuring the continuation of service provision at all times and referring to the Business Continuity Plan as and when required.
• Contributing to service development and participating in the projects
• Working alongside all partners as part of a multi-disciplinary approach
• Working closely with hospital team to facilitate hospital discharges which includes working from hospital
• Work on a flexible rota which requires evening, weekends and bank holiday working
Due to an expansion of the Community Response Service, we are looking for enthusiastic, caring, committed and reliable individuals to join our team
NVQ 2 or 3 is essential but if you have relevant background experience within a care provision, rehabilitation or commissioning service with a focus on person centred goal led outcomes we would like to hear from you.
It is an important role in the day to day running of the service which operates 365 days of the year from 7am to 10pm with a rolling rota shift pattern which includes regular weekends, early and late days shifts and bank holidays.
Successful applicants will be subject to an enhanced DBS check, paid for by Leicestershire County Council.
