We have a full-time temporary fixed term contract until March 2022 for a Community Support Worker post available in the Working Age Adults Disability Team in Hinckley.
We are looking form someone who can work under supervision to complete Care Act Assessments, ensuring a high quality, person-centred accountable statutory social care service is provided to adults, their families and their carers
Promote independence and facilitate solutions through self-directed support
Provide appropriate responses or referrals within required timescales, organisational, policy and legislative requirements
Identify, investigate and contribute to potential safeguarding, mental capacity, continuing health care and other high risk situations were an adults safety or liberty may be at risk
ESSENTIAL REQUIREMENTS:
- Relevant health/social care qualifications to NVQ level 3 or equivalent experience
- Relevant experience (paid employment, training or voluntary sector) working with adults in a social care setting
- Knowledge and understanding of the need to undertake need and risk assessments
- A good understanding of social care organisations and their roles and responsibilities as part of the wider community and voluntary sector
- Ability to remain up-to-date with social care policy and practice requirements
