The post holders will be responsible for providing high standards of direct care and support to service users who access the  residential, short breaks and community life choices  provision at the Trees.    
   
The post holder will plan monitor review and provide support packages to service users maintaining a high standard of support and care to people, ensuring we meet all their needs including emotional, physical, cultural and social. Promoting the rights and independence of the individual.     
   
ESSENTIAL REQUIREMENTS: 
• Good Organisational Skills
• Effective communication skills - Must be able to engage with service users, colleagues and our external partners
• Ability to write clear and concise records
• Able to assist with the moving and handling of service users
• Ability to manage workloads and prioritise
• Ability to remain calm under pressure
• Able to work alone or as part of a team.
• Able to work shifts including weekends and bank holidays
