Responsibilities of an APMH/Social Worker:
• You will need to be able to travel around the Charnwood area to support the working aged adults who need our care, this includes remote working when required.
• You will specialise in supporting people with significant/chronic/enduring mental illness and drug and alcohol issues.
• You are required to assess eligibility, care and support needs and where appropriate mental capacity in line with statutory policy and legislation.
• You will be required to identify when someone is at risk or is experiencing abuse and lead on our safeguarding duties.
• You will identify, commission and review services in line with eligibility and need whilst promoting independence.
•You will be required to work cohesively with other professionals (e.g. health services) to provide the best outcomes for our service users.
