Responsibilities of a Social Worker:
• You will need to be able to travel around the Charnwood area to support the working aged adults who need our care, this includes remote working when required
• You will specialise in supporting adults with learning and physical disabilities.
• You are required to assess eligibility, care and support needs and where appropriate mental capacity in line with statutory policy and legislation.
• You will be required to identify when someone is at risk or is experiencing abuse and lead on our safeguarding duties.
• You will identify, commission and review services in line with eligibility and need whilst promoting independence.
• You will be required to work cohesively with other professionals (e.g. health services) to provide the best outcomes for our service users.
• Our approach is person centred and we operate in multi-disciplinary teams making decisions together.
