An opportunity has arisen within the Working Age Adult Disability team based at Hinckley Hub for an experienced social worker to join the team. We are an established, friend team supporting people between 18-65 years old with complex disabilities, including learning disabilities, physical disabilities and ASD.
  
We are looking to recruit to two 37 hour, 12 month fixed term posts to cover maternity leave.
   
Working in Adult Social Care:

Salary Range: £23,868 - £33,972 pro rata
   
Work as a Social Worker delivering a high quality, asset based, person-centred, accountable statutory social care service, ensuring that adults and their families and carers;
• Are able to seek asset based solutions , assess, manage and review their own needs and risks, maximise their own independence and self-direct their responses to deliver agreed outcomes.
• Can access personalised care and support to meet their assessed needs and maintain themselves in the community. • Are protected and safeguarded.
  
ESSENTIAL REQUIREMENTS:
• Degree in Social Work approved by the Health Care Professions Council (HCPC) or equivalent social work qualification ie Certificate of Qualification in Social Work or Diploma in Social Work, approved by HCPC.
• The post-holder must be registered with the Health Care Professions Council and is responsible for maintaining registration.
• Evidence of effective assessment and risk management delivery.
• Evidence of prioritising work and meeting deadlines.
• Experience of partnership working with adults to identify and meet need with creative and pragmatic solutions.
• Evidence of working with those who access or need the service to identify issues and identify specific action.
• Post qualification experience in a social work setting.
• Experience of multi-agency/multidisciplinary working and/or of hospital discharge processes.
• Knowledge of; social work methodologies and ability to use them; Relevant legislation including The Care Act , Mental Health Act , Mental Capacity Act; The needs and issues of different adults including ethnic minorities, disability and age issues; Social Care policy such as personalisation, asset based social work, preventative strategies and solutions.
    
This post holder will provide robust and evidenced review of service users in high cost residential or supported living services, working alongside Health and Occupational Therapy professions to ensure effective care and support alongside reasonable costs for service provision for service users and improved working relationships with providers; moving forward to a service model to reflect increased independence for individuals and least restrictive practice in line with the Adult Social Care Strategy. Workers who secure these posts will have experience and confidence of working with service users who have complex needs, an understanding of the provider market and service options available, be able to work within a multi-disciplinary team to provide assessment, support planning and review functions, and have a desire to achieve good outcomes for service users and ensure cost effective service provision.
