A fantastic opportunity has arisen to recruit a full-time Social Worker within the Transitions Team, Adults and Communities.
The Transitions Team provides a countywide service to young people of Leicestershire with special educational needs and disabilities (SEND). The Care Act stipulates that preparation for adulthood should begin well before a young person reaches the age of 18. It is therefore crucial that the Transitions Team are able to begin the process of co-working and complete the Adult Social Care assessment and support plan at an appropriate stage prior to the young person’s 18th birthday.
Good preparation for adult life needs to start early and high aspirations are crucial to success. The focus should be on the young person’s strengths and capabilities and the outcomes they want to achieve.
To plan successfully, all services involved in the young person’s life need to actively engage and support the planning for adulthood. As a transition Social Worker an essential part of your role will be working collaboratively with children’s services, education and health.
To be considered for the Social Work role within the Transitions Team you will need:
- Degree in social work or equivalent
- Registration with Social Work England
- Experience of working within a Transitions team and/or Adult services
- Experience of assessing the needs young people with social care needs in line with the Care Act 2014 including Autistic Spectrum Disorder, mental health issues, and learning disabilities and their carers.
- To commission, monitor and review community support to young people and their carers in a way that maximises their independence and identify agreed outcomes
- Evidence an ability to effectively work with a complex caseload
- An ability to work predominantly under your own initiatives, with a level of autonomy, accountability and expertise that recognises when line management support is appropriate
- High quality problem solving and negotiation skills within a context of developing positive professional relationships with other agencies
- An ability to plan and act appropriately during situations or crisis management
- Appropriately supportive to lesser experienced staff
From the 1st April 2020, a Market Premia of £3,135 will be paid pro rata, to all level 3 social workers.
