The Whole Life Disability Programme supports citizens with physical and learning disabilities from childhood right through to adulthood to ensure a smooth transition between services. The team works closely with Health colleagues to commission and support citizens in a range of services including community support, residential and hospital settings.

Adult Social Workers provide support to vulnerable adults who are identified with a disability. They carry out assessments and reviews and then implement care and support according to their assessed needs and in line with current legislation. Adult Social Workers provide this support for young people from around 17, as they transition into adulthood and possibly require more long term support plans to be put in place. They will maintain professional relationships and support networks in place, as well as liaising with the Safeguarding team as and when appropriate.

Social Workers at Level 1 are required to undertake a full induction and complete a successful probationary period. Level 1 Social Workers would also be required to complete the ASYE within an agreed period. Social Workers at Level 1 are also expected to deal with safeguarding concerns, under the supervision of a more senior member of the team.

Social Workers at Level 2 and 3 are expected to carry a full caseload and to work with minimal supervision. Social Workers at Level 2 and 3 are also required to deal with safeguarding. Completion of ASYE (previously PQ consolidation award or Stage 1 Higher Specialist Award (Mental Health)) is required to practice at Level 2.

Essential Requirements

  1. CQSW / CSS / Dip SW, Degree Qualification and HCPC Registration;
  2. Knowledge and experience of relevant Social Care legislation and Government guidance;
  3. Commitment to Person-Centred Practice, as well as knowledge of and commitment to self-directed support;
  4. Experience of carrying a full caseload and undertaking assessments of need, with minimal supervision (L" and L3)
