Charnwood Older Adults Social Care team is a busy, well-established and friendly team, which offers a supportive environment for pro-active team players to learn and develop.  
          
We currently have the following vacancies:
1 x permanent community support worker
3 x temporary community support worker posts to 31st March 2020
         
Salary Details:
Scale point 9 £(24,801 to £26,925 per annum)
      
The daily activities of the role:
• Undertaking Care Act Assessments of the care and support needs of Older Adults and Carers.
• Managing a case load and prioritisation of that case load based on the known risks.
• Implementing support plans to meet unmet need.
  • Providing urgent /duty response
  • Facilitating hospital discharges
  • Undertaking Making Capacity Assessments
  • Working as a team member and as part of Multi-Disciplinary teams
  • Identifying and alerting safeguarding to mangers
  • Capturing assessments, support plans and case notes within the Adult Social Care systems in a timely, clear and concise manner
  
These posts are subject to an enhanced DBS check for a regulated activity.
