Do you have a passion for providing first class person centred care? We have a full-time permanent and a part-time permanent Community Support Worker post available within the Transitions Team, Adults and Communities. The Transitions Team provides a countywide service and is based in Hinckley Leicestershire.
As a Community Support Worker in the Transitions Team you will be preparing young people for adulthood, working collaboratively with children’s services, education and health.
You will work under supervision to deliver a high quality, person-centred accountable statutory social care service, ensuring that young adults, their families and carers; are able to find their own solutions, assess, manage and review their own needs, risks and capacity, maximise their own independence and self-direct the responses to deliver agreed outcomes.
Ensure young adults can access community opportunities.
You will formalise personalised care and support to deliver their expressed outcomes, maintain them in the community ad ensure they are protected and safeguarded.
