We have the following Fixed term Community Support Worker vacancies within our Countywide Residential Review service.
        
1 x 18.5 hour (Part time) Until 31st March 2022 Fixed term (Maternity cover)
1 x 18.5 hour (Part time) Until 31st August 2021 Fixed term (possibility of extension to end of March 2022) 
1 x 37 hour until 31st March 2022 Fixed term
    
Starting Salary £25,485 per annum (£12,742 Pro rata)
    
We are looking for confident and driven individuals who are committed to achieving excellent standards of service.
    
Working under supervision, completing annual Care Act review assessments for adults over 18 years of age, with eligible care and support needs. Ensuring a high quality, person-centred accountable statutory social care service is provided to adults, their families and their carers.  
    
Promoting independence and facilitating solutions through self-directed support.    
     
Providing appropriate responses or referrals within required timescales, organisational, policy and legislative requirements.    
    
Identifying and refering to potential safeguarding, contributing to mental capacity assessments, continuing health care, deprivation of liberty safeguards (DoLs) and other high risk situations were an adults safety or liberty may be at risk.    
     
ESSENTIAL REQUIREMENTS: 
  • Relevant health/social care qualifications to NVQ level 3 or equivalent experience
  • Relevant experience (paid employment, training or voluntary sector) working with adults in a social care setting
  • Knowledge and understanding of the need to undertake need and risk assessments
  • A good understanding of social care organisations and their roles and responsibilities as part of the wider community and voluntary sector
  • Ability to remain up-to-date with social care policy and practice requirements
