We are looking to recruit a Care Worker Level 3 to work within Social Care Reablement, for 37h/week on a permanent contract.
 
The starting salary is £22,183, rising to £23,541 on the first year anniversary, subject to satisfactory completion of a probation period (pro rata for part time workers).
 
Hours will be worked according to a rota, and this will include weekend work. A valid driving licence and access to your own vehicle is essential for this role.

Job Purpose
Social Care Reablement is a specialist Homecare team that works across the city to provide support to citizens in their own homes. This is mostly acute, short term care for citizens who have recently been discharged from Hospital or another service. The focus of the work done by Social Care Reablement is to help citizens reintegrate back into their previous lives, whether that is socially, physically or emotionally.
   
The team encourages independence and choice for citizens, and control over the way their care is provided. This involves not only providing personalised care as directed in the citizen’s care plan, but also encouraging and enhancing the wellbeing of citizens through promotion of independence, early intervention, community support and friendships.
   
Care Workers at L3 support colleagues at L2 to provide safe and appropriate personalised care and support to citizens. This includes writing and maintaining individual person centred care plans, completing assessments and the administration of medication.
   
The successful candidate will be required to engage in citizens’ meetings when needed, to ensure that the care package is always up to date and communicated to all relevant stakeholders as well as undertaking direct observations of care delivered, making recommendations of how to improve care provision and using feedback to adjust the care plan as required.
  
Essential Requirements
  1. Ability to assess social care and health needs and develop, implement, maintain and update person centred Care Plans;
  2. Ability to administer medication (create medication administration records, book in medication and complete returns and follow departmental policies and procedures) and conduct medication competency assessments;
  3. Excellent oral and written communication skills (including facilitating reviews, engagement, report writing, maintaining good quality records, letters, difficult conversations etc.)
  4. Knowledge and understanding of the rights, needs and feelings of vulnerable adults from diverse social and cultural backgrounds.
